If you have purchased a personalized website template ($279 version) please fill out the form below. Make sure you use the same email that you used when you purchased the website. The information you put on this form will be used to personalize your website template before it is sent to you. Anything left out will not be included on the template. Please double check form for accuracy and completion before submitting. Once submitted, your website will be sent via email within 7 days. Please note, if a question on this onboarding form is not included in the template it will not be added. For example, not every template will showcase team members. Additional edits and revisions are not included.
